General comments
Duur afhankelijk van het focus van de infectie.
Dosering vancomycine iv bij normale nierfunctie: 1000 mg 2dd (<70 kg) of 1000 mg 3 dd (>70 kg) met een eerste vancomycine dalspiegel op dag 2.
N.B. Bij gestoorde nierfunctie of tijdens niervervangede therapie, laat een vancomycine top- en dalspiegel bepalen aansluitend op de 1ste gift.
