General comments
Dosering vancomycine iv bij normale nierfunctie: 1000 mg 2 dd (<70kg) of 1000 mg 3 dd (>70kg) met een eerste vancomycine dalspiegel op dag 2.
N.B. Bij gestoorde nierfunctie of tijdens niervervangede therapie, laat een vancomycine top- en dalspiegel bepalen aansluitend op de 1ste gift.
Duur afhankelijk van het focus van de infectie.
Cave endocarditis bij enterokokken bacteremie zonder focus. NB: indien endocarditis, zie advies onder hoofdstuk endocarditis.
