General comments
Bij zeer goede respons eventueel switch naar oraal alternatief:
Vancomycine spiegelbepaling rondom derde gift. Afnametijdstip dalspiegel vlak voor de gift en topspiegel tenminste 2 uur na inlopen infuus. Afnametijdstip altijd noteren op aanvraagformulier.
N.B. Bij gestoorde nierfunctie of tijdens niervervangede therapie, laat top- en dalspiegel bepalen aansluitend op de 1ste gift.
