Indications: Natieve klep - MIC penicilline kleiner dan 0.1 mg/l
Priority	Medication	Remarks
	Medication: benzylpenicilline iv 2million. IU 6dd 2 weeks + gentamicine iv 3mg/kg 1dd 2 weeks	Remarks: Zie onderstaande opmerking voor de voorwaarden voor tweeweekse behandeling van vergroenende streptokokken.
	Medication: benzylpenicilline iv 2million. IU 6dd 4 weeks
	Medication: ceftriaxon iv 2000mg 1dd 2 weeks + gentamicine iv 3mg/kg 1dd 2 weeks	Remarks: Bij niet IgE-gemedieerde penicillineallergie. Zie onderstaande opmerking voor de voorwaarden voor tweeweekse behandeling van vergroenende streptokokken.
	Medication: ceftriaxon iv 2000mg 1dd 4 weeks	Remarks: Bij niet IgE-gemedieerde penicillineallergie.
	Medication: vancomycine iv 1000mg 2dd 4 weeks + gentamicine iv 3mg/kg 1dd 5 days	Remarks: Bij IgE-gemedieerde penicillineallergie. Let op: voorkeur voor penicilline desensibilisatie (op de IC) iom allergoloog en consulent infectieziekten.

	Medication: vancomycine iv 1000mg 2dd 4 weeks + gentamicine iv 3mg/kg 1dd 2 weeks	Remarks: Bij IgE-gemedieerde penicillineallergie. Let op: voorkeur voor penicilline desensibilisatie (op de IC) iom allergoloog en consulent infectieziekten.

Indications: Natieve klep - MIC penicilline groter dan 0.5 mg/l
Priority	Medication	Remarks
	Medication: amoxicilline iv 2000mg 6dd 4 to 6 weeks + gentamicine iv 3mg/kg 1dd 4 weeks
	Medication: vancomycine iv 1000mg 2dd 4 to 6 weeks + gentamicine iv 3mg/kg 1dd 4 weeks	Remarks: Bij penicillineallergie of penicilline hoog resistent (MIC penicilline groter dan 2.0 mg/l).

Voorwaarden voor tweeweekse behandeling van viridans streptokokken en S. bovis (bron: SWAB Guideline VII - Infectieuze endocarditis)

1. MIC penicilline < 0,1 g/ml

2. geen contra-indicaties voor aminoglycosiden

3. geen cardiovasculaire risicofactoren zoals hartfalen, aorta-insufficiëntie of geleidingsstoornissen

4. geen trombo-embolische complicaties

5. natieve klep

6. geen vegetaties > 5 mm

7. klinische respons binnen 7 dagen

Voor spiegelcontrole bij Vancomycine en Gentamicine: zie tabblad ''overig''

Eerste gentamicine dalspiegel voor gift op dag 3 (streefwaarde: <1 mg/L)

Behandelduur in overleg met internist-infectioloog / arts-microbioloog
