Indications: Kunstklep - MIC penicilline kleiner dan 0.1 mg/l
Priority	Medication	Remarks
	Medication: benzylpenicilline iv 2million. IU 6dd 6 weeks
	Medication: ceftriaxon iv 2000mg 1dd 6 weeks	Remarks: Bij niet IgE-gemedieerde penicillineallergie.
	Medication: vancomycine iv 1000mg 2dd 6 weeks + gentamicine iv 3mg/kg 1dd 2 weeks	Remarks: Bij IgE-gemedieerde penicillineallergie. Let op: voorkeur voor penicilline desensibilisatie (op de IC) iom allergoloog en consulent infectieziekten.

≥ 18 years

	Medication: vancomycine iv 1000mg 2dd 6 weeks + gentamicine iv 3mg/kg 1dd 6 weeks	Remarks: Bij penicillineallergie of penicilline hoog resistent (MIC penicilline groter dan 2.0 mg/l).
